- If you are an existing customer, download and complete the Scheme Member's Request for Fund Transfer Form (INPM)
- If you are a new customer, download and complete the Scheme Member's Request for Fund Transfer Form (INPM) and Preserved Account Holder Application Form (IN71 - SuperTrust, IN81 - SuperTrust Plus or IN77 - SimpleChoice (New))
