All forms or written notices must be signed by at least one authorised person designated on the Authorised Signatures Specimen form to give instructions for the corresponding transactions. If you have not yet submitted the above form to us, the authorised person will be the party who signed the Employer Application Form.

New / Change of Direct Debit Instruction Notification Form

Direct Debit Authorisation

Notify an addition or change to a direct debit authorisation for your MPF scheme or a particular pay centre, and authorise deduction of MPF contributions from your bank account. These two forms must be submitted together.

Paying your contributions through direct debit saves you time in writing and checking the payee and total amount for each payment.

Please note that about two weeks' processing time is required to set up a direct debit authorisation for a bank account with HSBC or Hang Seng Bank, or about a month for other bank accounts.

Authorised Signatures Specimen
Notify or change the signature specimen of your authorised persons and their authority level. At least one person must be authorised for each type of transaction:
  • Contribution payments, ie submit a Remittance Statement and report employee termination
  • Benefit payments, ie approve employee's partial withdrawal request (where employer's approval is required due to contractual arrangements) and supporting documents for claims
  • Reserve account fund switching, ie change investment allocation for your reserve account

Change of Member's Employment Details Notification Form
Notify any change to your employees' pay centre, class or member type.
INY2
Member Transfer Form
Transfer employees between companies whereby the new employer agrees to recognise the employee's length of employment with the existing employer for the purpose of making a Long Service Payment or Severance Payment.
INET
Participating Employer's Request for Fund Transfer Form
Notify any transfer of employees to another company's MPF scheme due to a change in business ownership or transfer of employment between companies. This form should be submitted within the permitted period together with the Member Transfer Form (INET). Please see Intra-Group Transfers for details to ensure that the appropriate procedures are followed.

This form is also applicable for transferring an MPF scheme between different MPF service providers.

Pay Centre Set Up Form
Set up different pay centres for employees with different payroll cycles. It is also applicable for setting up different pay centres for enrolling casual and non-casual employees in order to separate the remittance of contributions due to different payment deadlines under the MPF legislation.

Asset Switch of Existing Balance of Reserve Account Form
Switch all or some of the units from one fund or funds in your reserve account and then reallocate the redeemed amount to another fund or across any of the other funds. Any future amounts transferred into the account will continue to follow your latest investment allocation instruction.

Remittance Statement - Daily Contribution
Report relevant income and contributions for daily paid casual employees.

Duplicate Remittance Statement Requisition Form
Request a duplicate copy of any previous Remittance Statements (up to the previous seven years).

The handling charge is HKD50 per statement (minimum HKD200 per request).
